it's not an artwork bug, but a packaging one.

so what do you mean with using 'gtk-qt engine', you have gtk-qt-engine installed ?
if yes does it work better without it

what happen it you remove the files:
 /home/£USER/.gtkrc-2.0-kde
 /home/$USER/.config/gtk-3.0/settings.ini
 /home/$USER/.kde4/env/gtk-qt-engine.rc.sh

yes, i have gtk-qt-engine installed. i deleted the first and the last, the folder gtk-3.0 doesn't exist (only 2.0). nothing's changed. can i do anything else? thank you Manuel

Missing icons are in packages gnome-icon-theme-symbolic
(the icons have no equivalent in any other icon theme, due to their name being hard coded in totem)
In principle oxygen-gtk3 should fall back on it, except maybe if oxygen-gtk3 was compiled with flag
OXYGEN_FORCE_KDE_ICONS_AND_FONTS=0 (remember reading something like that in mageia's mailing list).

In any case, this is a duplicate.
