It is not the 'Settings Manager' which lacks an icon; these are shown highlighted green, and are correct.
I assume the placeholders icons referred to are those highlighted red; the lower one normally translates as 'Preferences' (but so would 'Settings'!).

theme:Adwaita , application launcher menu missing 2 menu icons, placeholders instead also effect is evident in Cinnamon "recent files". changing title This is really hard to fix as GNOME / Adwaita doesn't fully follow freedesktop.org Icon Naming Specification [1] anymore. We had similar issues in the past with Oxygen icons and it was a big mess with all those symlinked icons to fix missing "standard icons". According to the specification it "describes the standard icon names that should be used by artists when creating themes, and by developers when writing applications which will use the Icon Theme Specification." One of the "Standard Category Icons" described in the specification is preferences-system that is used for example in Xfce's Settings menu [2]. Adwaita doesn't provide preferences-system icon anymore. GNOME uses nowadays "symbolic" style icons, but not all non-GNOME apps or desktop environments are supporting "symbolic" icons and fallbacking automatic to "traditional" icons or vice versa.
