In squid-cachemgr, reached after installing it by browsing to 
http://localhost/cgi-bin/cachemgr.cgi the top link titled Cache Manager Interface gives an error when clicked..

 Internal Error: Missing Template MGR_INDEX

All the other links seem to display meaningful data but it is very plain html tables etc obviously with no template.

See also bug 12914 which seems to suggest there should be icons and a template applied.

I feel dirty doing this, but this is INVALID.  Apparently this is by design and you have to install third-party Squid Cache Manager addons that implement the MGR_INDEX template.  I don't know why they don't just remove the "Interface" link from the page when you don't have one installed.  So, this turns out to be unrelated to Bug 12914 unfortunately.
